Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members in their residences Conduct thorough reviews of medical history, current conditions, and medication usage Educate patients on their health conditions and medication regimens Identify care gaps and provide actionable recommendations to close these gaps No requirement to develop treatment plans or prescribe medication

Qualifications:

Active Nurse Practitioner license in Mississippi (MS) Board Certification through AANP or ANCC required MSN degree from an accredited nursing program Current CPR certification (BLS) At least six months of recent Nurse Practitioner experience preferred
