Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Thoroughly review and evaluate prior and current medical history, existing conditions, and medication use Educate patients about their medical conditions and medications Identify gaps in care and make recommendations to resolve them Focus solely on assessmentno need to create treatment plans or prescribe medications

Qualifications:

Current and unrestricted Nurse Practitioner license in Mississippi Board certification through AANP or ANCC required Active BLS/CPR certification MSN degree from an accredited nursing program At least 6 months of recent Nurse Practitioner experience is strongly preferred
