Schedule:

Flexible shifts available seven days a week Ability to set your own schedule Minimum commitment per week or every two weeks

Responsibilities:

Conduct annual or post-discharge home health risk assessments, each typically lasting about 45 minutes Review and evaluate patients prior/current medical history, ongoing medical conditions, and medication usage Educate individuals about their chronic conditions and prescribed medications Identify care gaps and offer recommendations to improve patient outcomes No responsibility for treatment plan creation or medication prescribing

Qualifications:

Active Nurse Practitioner License in Mississippi Current CPR (BLS) certification Board Certified via AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of current NP experience preferred This position offers compensation per completed evaluation, ranging from $80 to $90 each, along with a mileage stipend and reimbursement for certain administrative tasks.
