The Wound Care Nurse plays a vital role in improving resident outcomes through the assessment, treatment, prevention, and management of acute and chronic wounds. Working collaboratively with physicians, nursing staff, therapy, dietary services, and the interdisciplinary team, the Wound Care Nurse develops and implements individualized treatment plans while ensuring compliance with federal and state regulations, infection prevention standards, and facility policies. Position Summary The Wound Care Nurse is responsible for coordinating and overseeing the facility's wound care program to promote optimal healing, prevent complications, and reduce the incidence of avoidable pressure injuries. This position serves as the clinical resource for wound management and provides education, consultation, and support to nursing staff regarding best practices in skin integrity and wound prevention. Essential Duties and Responsibilities Perform comprehensive skin and wound assessments upon admission and as clinically indicated. Evaluate, stage, measure, photograph (per facility policy), and document wounds accurately and timely. Develop, implement, and update individualized wound treatment plans in collaboration with the attending physician and interdisciplinary team. Perform or oversee wound treatments according to physician orders and evidence-based practices. Monitor healing progress and recommend modifications to treatment plans as appropriate. Conduct weekly wound rounds and participate in interdisciplinary care conferences. Identify residents at risk for pressure injuries and implement preventive interventions. Collaborate with dietary services to address nutritional factors affecting wound healing. Coordinate with rehabilitation services regarding positioning, mobility, and pressure redistribution strategies. Ensure appropriate specialty mattresses, cushions, positioning devices, and other pressure-relieving equipment are utilized. Monitor residents for signs of infection and communicate changes in condition promptly to providers. Educate residents and families regarding wound prevention and treatment plans. Provide ongoing education and competency training for nursing staff related to: Pressure injury prevention Skin assessments Wound treatment techniques Infection prevention Documentation requirements Ensure compliance with physician orders, facility policies, CMS regulations, and state requirements. Maintain accurate wound logs, treatment records, and quality improvement data. Participate in Quality Assurance and Performance Improvement (QAPI) activities related to skin integrity and wound care. Assist with survey preparation and respond to regulatory inquiries related to wound management. Collaborate with pharmacy regarding medications that may impact wound healing. Participate in infection prevention initiatives and antibiotic stewardship efforts. Perform other nursing duties as assigned. Qualifications Required Current unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Montana. Current CPR/BLS certification. Minimum one year of nursing experience, preferably in long-term care, rehabilitation, or wound care. Knowledge of pressure injury staging, wound assessment, and evidence-based treatment practices. Strong assessment, documentation, and communication skills. Ability to work independently while collaborating effectively with the interdisciplinary team. Preferred Wound Care Certification (WCC, WOCN, CWCN, or equivalent). Current unrestricted Registered Nurse (RN) Experience in skilled nursing or post-acute rehabilitation. Knowledge of CMS regulations, PDPM, and long-term care documentation requirements. Experience participating in QAPI initiatives and survey readiness. Knowledge, Skills, and Abilities Thorough understanding of wound healing principles and pressure injury prevention. Excellent clinical assessment and critical thinking skills. Strong organizational and time management abilities. Proficiency with electronic medical records. Ability to educate and mentor nursing staff. Commitment to resident-centered, compassionate care.
