As an RN Case Manager, you'll provide personalized care, coordinate services, advocate for patients, and support families through every stage of their healthcare journey. You'll enjoy the independence of caring for patients in their homes while benefiting from the support of an experienced interdisciplinary team. What You'll Do As an RN Case Manager, you will: Assess patient needs and develop individualized plans of care Coordinate services with physicians, therapists, caregivers, and other healthcare professionals Manage and monitor patient caseloads Provide education and support to patients and families Ensure quality outcomes through ongoing assessments and care planning Promote patient comfort, independence, and overall well-being Deliver compassionate care through both Home Health and Hospice services Qualifications Qualified candidates will possess: Current Registered Nurse (RN) license Current CPR certification Strong assessment, critical thinking, and clinical decision-making skills Excellent communication and care coordination abilities Ability to work independently and manage multiple priorities Commitment to providing patient-centered care Passion for serving patients and families with compassion and respect Why Join Brookestone Home Health & Hospice?
