Nurse Practitioner Long-Term Care New Hampshire We are seeking a Nurse Practitioner to join a facility-based medical team providing comprehensive primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively within an interdisciplinary care team. Responsibilities Provide primary and preventive care to long-term care residents Manage residents with acute changes in condition Perform advanced care planning and chronic care management Prescribe medications, including controlled substances, in accordance with NH law Order, review, and follow up on diagnostic testing and treatments Communicate regularly with nursing facility staff regarding patient condition and care plans Collaborate with physicians and interdisciplinary care teams Schedule Monday Friday, 8:00 AM 5:00 PM EST 4 days per week (schedule coordinated with manager) Qualifications Current, unrestricted Nurse Practitioner license in New Hampshire Board Certified NP Active DEA license Background in adult, family, or geriatric medicine Experience in a long-term care or skilled nursing setting preferred Proficiency with EMR systems Strong clinical judgment managing complex, chronic conditions Benefits Continuing education credits Recertification reimbursement Malpractice insurance Health, dental, and vision insurance Paid time off
