Key Responsibilities:

Complete home health risk assessments, typically lasting 45 minutes per visit Review and evaluate prior/current medical history, underlying conditions, and medication lists for each member Deliver clear education regarding diseases and medication routines Detect potential gaps in care and suggest actions to support resolution Participate in a variety of visits, including annual health assessments and post-discharge follow-ups No requirement for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in NH Current Basic Life Support (BLS/CPR) certification Board Certification through AANP or ANCC Minimum of 6 months NP experience preferred MSN from an accredited nursing program This role offers a per evaluation compensation of $80-$90, in addition to a mileage stipend and pay for administrative time.
