Key Responsibilities:

Complete in-home risk assessments and health evaluations for Medicare Advantage members Thoroughly review and assess medical histories, existing conditions, and medication regimens Provide clear education on diagnoses and medications to patients and their families Identify gaps in care and recommend actions or follow-ups to improve patient outcomes Maintain comprehensive documentation following each assessment

Qualifications:

Active Nurse Practitioner license in NH Current CPR certification (BLS) Board Certified through AANP or ANCC Masters degree in Nursing (MSN) from an accredited institution At least 6 months of recent Nurse Practitioner experience preferred This position provides excellent earning potential of $80-$90 per completed evaluation, with additional support for mileage and paid administrative time.
