Responsibilities:

Complete annual home health risk assessments and post-discharge visits for Medicare Advantage members residing throughout Rockingham County Conduct comprehensive reviews of prior and current medical histories, conditions, and medications during 45-minute in-home evaluations Educate patients on their health status, medications, and self-care Identify care gaps and communicate clear recommendations to patients and healthcare teams Administrative responsibilitiesno creation of treatment plans or medication prescribing

Qualifications:

Current Nurse Practitioner (NP) license in New Hampshire Board certification through AANP or ANCC Active CPR (BLS) certification MSN from an accredited nursing school At least 6 months of recent NP experience preferred Prior home health or risk assessment experience a plus Enjoy competitive compensation of $80$90 per evaluation, plus a mileage stipend and consideration for administrative time.
