Our Home Health Registered Nurses enjoy some notable benefits: Competitive pay and flexible scheduling options One-on-one patient care that allows you to build meaningful relationships Greater autonomy and work-life balance Online training, professional development, and CEU opportunities Supportive clinical leadership and team collaboration Opportunities for career growth and advancement Health, dental, vision, PTO, and retirement benefits for eligible employees As a Home Health Registered Nurse, here's a big-picture view of what you'll do: Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans Perform comprehensive assessments and ongoing evaluations of patients with acute illnesses, chronic conditions, injuries, and post-surgical needs Develop, implement, and update patient care plans to promote optimal health outcomes Administer medications, treatments, wound care, and other skilled nursing interventions Monitor patient progress, identify changes in condition, and communicate findings to physicians and interdisciplinary team members Educate patients and caregivers regarding disease management, medications, treatments, nutrition, and safety practices Coordinate care with physicians, therapists, social workers, and other healthcare professionals Supervise and collaborate with Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other support staff as appropriate Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements Participate in case conferences, quality improvement activities, and discharge planning to ensure continuity of care A few must-haves for
Home Health Registered Nurses:
Graduate of an accredited nursing program Active Registered Nurse (RN) license in the State of New Hampshire Minimum of one (1) year of nursing experience; home health, hospice, rehabilitation, or medical-surgical experience preferred Current CPR certification Valid driver's license, reliable transportation Knowledge of state and federal home health regulations and standards of practice Strong clinical assessment, critical thinking, and care coordination skills Excellent communication, documentation, and interpersonal skills Compassionate approach to patient-centered care and family education
Preferred Qualifications:
