Job Description

40 hours / week: Monday - Friday 8:00am - 4:30pm. No weekends, but must work one winter and one summer holiday each year. Primary territory would be Nashua/Hudson. Secondary focus would be the Londonderry, Derry, Windham, Salem, Pelham area. About the
Job:
Admission to HHHC's homecare division. Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Utilizes the nursing process in planning, providing, and evaluating skilled needs. Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring. Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
Who You Are:
Graduate of an accredited school of nursing. Associates degree in nursing. Registered as RN with New Hampshire's Board of Nursing. A minimum of one (1) year recent acute care experience. Ability to exercise initiative and independent judgment. Excellent verbal and written communication skills and competent in problem solving abilities.
