In this flexible role, you will conduct home health evaluations for Medicare Advantage plan members, supporting the goal of proactive, quality patient care.

Key Responsibilities:

Perform annual home health risk assessments on Medicare Advantage members Review and evaluate patients' prior and current medical histories, medical conditions, and medication use Educate patients on their specific medical conditions and prescribed medications Review care gaps, make recommendations for resolution, and document findings appropriately No treatment planning or medication prescribing required

Qualifications:

Current Nurse Practitioner license in New Jersey Active CPR certification (BLS required) Board Certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program Those selected benefit from per evaluation compensation ($80$90 per completed assessment) along with a mileage stipend and payment for administrative time.
