Overview Nurse Time Homecare is a New Jersey-licensed home care organization dedicated to helping clients remain safe, comfortable, and independent in their homes. We are seeking a compassionate and dependable Registered Nurse Clinical Supervisor to conduct client assessments, develop individualized plans of care, supervise home care services, and support compliance with New Jersey requirements and CAHC accreditation standards. This position includes a combination of in-home visits, clinical documentation, caregiver supervision, telephone follow-up, and administrative responsibilities. Hours will initially be based on client volume, with the potential to increase as the agency grows. Responsibilities Conduct initial in-home nursing assessments before services begin. Evaluate each client's physical, functional, social, environmental, and personal care needs. Develop, sign, implement, review, and revise individualized plans of care. Ensure caregiver assignments are appropriate for each client's assessed needs. Complete required 30-day supervisory contacts and document whether the plan of care continues to meet the client's needs. Conduct required 60-day in-home reassessments and supervisory visits. Reassess clients promptly following a significant change in condition, hospitalization, incident, or change in service needs. Monitor caregiver performance and compliance with the client's plan of care. Review caregiver notes, service records, incident reports, complaints, and other clinical documentation for completeness and accuracy. Communicate with clients, family members, caregivers, physicians, and other healthcare professionals when appropriate. Provide clinical guidance and support to Certified Homemaker-Home Health Aides and other direct-care staff. Participate in caregiver orientation, education, competency evaluation, and ongoing supervision. Assist with infection control, emergency preparedness, client safety, complaint management, incident review, and performance-quality improvement activities. Maintain accurate, timely, confidential, and survey-ready clinical records. Follow HIPAA requirements and all applicable federal, New Jersey, agency, and accreditation standards. Participate in staff meetings, case conferences, accreditation preparation, and quality-improvement activities. Remain reasonably available for clinical consultation and urgent client-care concerns according to the agency's on-call procedures. Perform other nursing and supervisory duties within the RN's scope of practice and agency policies. Qualifications Active and unrestricted New Jersey Registered Professional Nurse license in good standing. Current CPR/ Basic Life Support certification. Minimum of one year of professional nursing experience preferred. Home care, community health, geriatric, case-management, or supervisory nursing experience strongly preferred. Knowledge of client assessment, care planning, clinical documentation, infection control, and caregiver supervision. Familiarity with New Jersey home care requirements and accreditation standards is preferred. Strong assessment, clinical judgment, organization, communication, and documentation skills. Ability to work independently while collaborating effectively with agency leadership, caregivers, clients, and families. Compassionate, dependable, respectful, and committed to client-centered care. Reliable transportation, a valid driver's license, and willingness to travel to clients' homes throughout South Jersey. Ability to complete required background checks, reference verification, health screening, drug screening, and employment documentation. Basic computer skills and the ability to complete electronic or paper clinical records accurately and on time.
