RN Transitional Care Be the First to Apply Job Description We are seeking a dedicated and motivated Registered Nurse (RN) to join our team at AtlantiCare Health Engagement, Inc. in the Transformational 117 department. In this role, you will play a crucial part in supporting patients post-hospitalization, ensuring a seamless transition back to their daily lives. Your expertise will be embedded within the Atlanticare Primary Care Physican Group, where you will provide invaluable assistance to patients, helping them navigate their care journey. Responsibilities Assist patients post-hospitalization with appointment scheduling and medication management. Educate patients on their disease process and provide comprehensive health education. Identify and address concerns related to the patient's diagnosis and social determinants of health (SDOH). Prevent readmissions by proactively addressing patient needs and providing necessary support. Collaborate with the Atlanticare Primary Care Physican Group to ensure a coordinated care approach. Maintain accurate and up-to-date patient records and documentation. Communicate effectively with patients, their families, and the healthcare team to ensure a holistic approach to care. Stay informed about the latest advancements and best practices in transitional care. Provide emotional support and guidance to patients during their recovery process. Qualifications Registered Nurse (RN) license in good standing. A minimum of 2 years of experience in a healthcare setting, preferably in transitional care or primary care. Excellent communication and interpersonal skills, with the ability to build rapport with patients. Strong organizational and time management skills, with the ability to prioritize tasks. Empathy and a patient-centered approach to care. Ability to work independently and as part of a multidisciplinary team. Proficiency in using electronic health records (EHR) systems. Flexibility to work remotely and in-office as needed. Current CPR certification is preferred. A passion for patient advocacy and a commitment to continuous learning.
