Responsibilities:

Patient Care:

Deliver high-quality nursing care during after-hours, addressing physical, emotional, and educational needs.

Emergency Response:

Respond promptly to calls, conduct home visits as needed, and manage urgent patient needs.

Symptom Management:

Assess and manage pain and other symptoms to promote patient comfort.

Family Support:

Provide guidance and reassurance to families during challenging situations.

Collaboration:

Communicate effectively with physicians, case managers, and interdisciplinary team members.

Documentation:

Maintain accurate, timely records of all patient care and communications. Adhere to

Qualifications:

Education:

Current NJ RN license required.

Experience:

1-2 years of home care, hospice, or acute care experience preferred.

Skills:

Excellent clinical assessment, critical thinking, and communication skills.

Availability:

Ability to work night shifts, including weekends and holidays as scheduled.

Transportation:

Reliable vehicle, valid driver's license, and proof of auto insurance.

Working Conditions/Physical Demand:

Combination of indoor and outdoor work; includes patient contact, use of clinical equipment, and electronic documentation.

Compensation:

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is $46.96- $58.43 , which reflects base pay, overtime, bonuses, and other incentives. Actual compensation will depend on experience, skills, and internal equity.
