Primary Responsibilities:

Perform annual risk assessments in home settings for Medicare Advantage members Thoroughly review and evaluate patients medical history, conditions, and medication usage Educate individuals about their medical conditions and medication management Detect any gaps in care and offer actionable recommendations to resolve them No medication prescribing or treatment planning required

Desired Qualifications:

Active Nurse Practitioner license in NJ Current CPR certification (BLS) Board certification through AANP or ANCC required At least six months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing program Enjoy a pay structure of $80$90 per evaluation, complemented by a mileage stipend and compensation for administrative time.
