Lawrenceville, NJ About the Role Lead the clinical heart of our home care operation — setting the standard for client safety and care quality, and building relationships with clients, families, and healthcare partners that keep people well cared for at home. If you're a strong clinical leader who wants real ownership over how care is delivered, this is your seat at the table.
What You'll Own Clinical Leadership:
Oversee client care from initial safety assessments through ongoing care plan updates as conditions change.
Relationships:
Build strong ties with clients, families, caregivers, and healthcare partners, including hospital/rehab visits for new, transitioning, or high-risk clients.
Retention & Growth:
Spot at-risk clients early and address concerns; identify where additional care could help existing clients.
Quality & Compliance:
Ensure visits, assessments, and documentation meet company and regulatory standards; support caregiver training and provide clinical guidance as needs change.
Coordination:
Partner with Social Work, Client Care, Scheduling, and Operations; lead weekly reviews on at-risk clients and growth opportunities.
Responsive Support:
Resolve grievances and clinical concerns, including occasional after-hours support, and participate in annual accreditation audits. What You Bring Active NJ RN license in good standing. Strong clinical judgment and the ability to juggle multiple client situations. Confident, natural relationship-builder across clients, families, facilities, and internal teams. Comfort with a field-based role balanced with office time for oversight, documentation, and coordination. Flexibility for occasional after-hours/weekend support.
