As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients. This opportunity offers $38.00 - $45.05 an hour based on experience! What You'll Do:

Perform patient assessments for comfort and relief symptoms Establish and implement an individualized plan of care for each patient Assess the need for patient and family education and provide education as needed. Obtain appropriate physician orders to achieve symptom management Triage and arrange home visit schedule, demonstrating efficiency in time and mile management Make referrals to additional IDT members and collaborate with social workers related to their involvement. Our Team:

Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred N. required; BSN Degree preferred CHPN or OCN preferred, BLS training NJ driver's license and insurance coverage required Bilingual is a plus!
