Key Responsibilities:

Coordinate patients throughout their acute care hospitalization Manage all aspects of discharge planning and transitions of care Perform utilization management and quality screenings Collaborate with patients, families, providers, and the healthcare team Ensure appropriate resources and services are arranged for discharge

Requirements:

2+ years of acute care experience strongly preferred Inpatient Case Management experience

• Required Experience in Case Management and Managed Care BSN
• Required Active RN License
• Required

CCM/CCMC
