Monday•Friday 8:00 AM•5:00 PM No on-call requirements

30 points per week Mileage reimbursement: $0.43/mile Field-based role

Coverage Area:

Passaic County, NJ Key Responsibilities:

Perform comprehensive assessments of high-risk patients Develop and implement individualized care plans in collaboration with primary care providers Coordinate care transitions and interdisciplinary communication Monitor patient progress and adjust care plans as needed Identify cost-effective and appropriate levels of care Coordinate non-medical support services (e.g., housing, transportation) Maintain accurate documentation of clinical and functional outcomes Promote health education, illness prevention, and patient safety Advocate for patient privacy, confidentiality, and quality of care

Required Qualifications:

Active RN License (New Jersey) Associate's or Bachelor's degree in Nursing Strong clinical assessment and decision-making skills

Preferred Skills:

OASIS experience Home Care Home Base (HCHB) experience Case Management certification

Pay:

$45.00•$52.00 per hour Expected hours: 40.0 per week
