Elevate patient care in Rio Arriba County by providing in-home risk assessments as a Nurse Practitioner specializing in home health. In this per diem position, youll have the autonomy to create your own schedule with flexible shifts available between Monday and Sunday, meeting a minimum weekly or bi-weekly hourly commitment. Assessments are performed in patients homes across Espanola, Chama, Abiqui, and surrounding areas.
Key Responsibilities:
Perform annual home health risk assessments for Medicare Advantage plan members Evaluate patients prior and current medical history, medical conditions, and medication use Educate patients on their medical conditions, prescribed therapies, and effective medication management Detect gaps in care and make informed recommendations for follow-up or additional resources No requirement for prescribing medications or developing treatment plans

Qualifications:
Active Nurse Practitioner license in New Mexico MSN degree from an accredited nursing program Current CPR certification (BLS) Board Certification through AANP or ANCC required At least 6 months of experience as a Nurse Practitioner preferred
