Key Responsibilities:

Conduct thorough, approximately 45-minute home health risk assessments for Medicare Advantage members Evaluate patient histories, existing medical conditions, and medication use Deliver proactive health and medication education tailored to each individual Pinpoint care gaps and recommend appropriate actions No requirements for creating treatment plans or prescribing medications

Qualifications:

Nurse Practitioner License in New Mexico Board Certified by AANP or ANCC Current BLS/CPR certification MSN from an accredited nursing program At least 6 months of recent NP practice (preferred) This role offers compensation of $80-$90 per completed evaluation, including a mileage stipend and a paid administrative time component.
