Key Responsibilities:
Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review prior and current medical history, chronic conditions, and medications with patients Educate individuals about their health conditions and corresponding treatments Detect care gaps and recommend appropriate solutions No development of treatment plans or prescribing responsibilities required
Qualifications:
Active Nurse Practitioner license in New Mexico Current BLS (CPR) certification Board certification via
AANP or ANCC Preferred:
