This position centers on fostering patient-centered care, ensuring continuity, and upholding the highest clinical standards. Schedule & Assignment Details:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Caseload of 2532 points per week (average 35 visits each day) Field-based in patients homes, with daily local travel (mileage reimbursed)

Key Responsibilities:

Complete thorough OASIS assessments, establishing and managing individualized care plans Deliver skilled nursing wound care, IV therapy, medication management, and chronic disease support Communicate promptly with the care team and physicians about changes in patient conditions Educate patients and families on health management, treatments, and medications Document care accurately and promptly using the Home Care Home Base (HCHB) system Collaborate with physical and occupational therapists, medical social workers, and aides for coordinated care Participate in interdisciplinary meetings, ongoing education, and compliance with agency and regulatory standards

Qualifications:

Active RN license in the practicing state, in good standing Minimum of one year of recent field-based Home Health RN experience OASIS assessment and HCHB documentation proficiency required Strong communication, assessment, critical thinking, and time management ability Reliable transportation, valid drivers license, and current auto insurance BSN and experience managing a 2532 point caseload preferred Familiarity with Medicare and home health compliance is highly valued
