Experienced Nurse Practitioners are sought for per diem home health roles conducting risk assessments for Medicare Advantage plan members in Albuquerque, Los Ranchos de Albuquerque, Tijeras, and other areas of Bernalillo County. Key Responsibilities:

Perform annual risk assessments in-home for Medicare Advantage members Assess patients medical and medication history, and document findings Educate patients about their conditions, medications, and healthy living strategies Identify potential gaps in care and make recommendations to close them Complete each assessment in approximately 45 minutes No prescribing or treatment plan creation required

Qualifications:
