Opportunity available for a skilled Nurse Practitioner to perform home health risk assessments on a per diem basis. This role involves traveling throughout Santa Fe County, NMincluding Santa Fe, Espanola, Edgewood, and nearby areasto visit Medicare Advantage plan members in their homes. Assessments typically last about 45 minutes and may include annual health evaluations or post-discharge visits. Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members. Review patients prior and current medical and medication history. Educate patients about their medical conditions and prescribed medications. Identify and document gaps in care, providing advice and recommendations for improvement. No responsibilities for prescribing medications or creating treatment plans.

Qualifications:

Active Nurse Practitioner license in New Mexico (NM) Board Certification through AANP or ANCC Current Basic Life Support (BLS) certification Master of Science in Nursing (MSN) from an accredited program At least 6 months of recent Nurse Practitioner experience preferred In this per diem position, you are compensated $80$90 per evaluation, with additional stipends for mileage and administrative time.
