Responsibilities:
Conduct detailed annual risk assessments in members homes Review and document current and prior medical history, conditions, and medication usage Provide tailored education about health conditions and prescribed medications Identify and relay any potential gaps in care and recommend solutions No medication prescribing or treatment plan creation required
Qualifications:
Active Nurse Practitioner license in NM Current BLS (CPR) certification Certification through AANP or
ANCC MSN
