Schedule & Details:

Full-time, 13-week contract Monday Friday, 8:00 AM to 5:00

PM Caseload:

2532 points per week (typically 35 patient visits per day) Field-based with daily travel and mileage reimbursement

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing care such as wound management, IV therapy, medication administration, and chronic disease interventions Closely monitor and document patient progress using the Home Care Home Base (HCHB) platform Educate patients and caregivers on care techniques, medications, and preventative health practices Collaborate with interdisciplinary teams (PT/OT, social workers, aides) to coordinate holistic care Actively participate in case conferences and staff development activities Ensure compliance with regulatory and agency standards in documentation and direct patient care

Qualifications:

Active RN license in the state of practice and in good standing Minimum one year of recent field Home Health RN experience Proficient with Home Care Home Base (HCHB) and OASIS assessments Strong clinical assessment, independent decision-making, and organization skills Reliable transportation, valid drivers license, and auto insurance BSN and experience with Medicare guidelines preferred
