Role:

We are seeking a highly skilled and dedicated RN MDS Coordinator to join our team at Mission Pines Nursing & Rehab Center. As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring that our residents receive the necessary care and services to meet their unique needs. Responsibilities:

Coordinate and manage the MDS assessment process, including identifying and addressing any issues or concerns Collaborate with the healthcare team to ensure that all assessments are completed accurately and efficiently Develop and implement individualized care plans that meet the specific needs of our residents Provide education and support to both residents and their families to promote independence and quality of life Identify and address any potential quality of care issues, ensuring that our residents receive the highest level of care Maintain accurate and detailed records of all assessments, care plans, and resident progress

Requirments:

RN license in Nevada MDS experience MDS license in Nevada preffered RN long-term experience
