Job Description

CareScout is seeking a compassionate Registered Nurse to guide families through complex long-term care decisions for aging loved ones. In this role, you'll assess care needs, interpret clinical information, and translate it into clear, personalized recommendations. You'll collaborate with care advocates, data and provider networks to coordinate safe, appropriate care options and monitor outcomes. Ideal candidates bring strong clinical judgment, excellent communication skills, and a passion for advocacy, helping families feel informed, confident, and supported at every step of the care journey. Responsibilities Conduct comprehensive nursing assessments of older adults' health and functional status Develop and refine personalized long-term care recommendations for families Educate families on care options, risks, and benefits in clear, empathetic language Collaborate with care advocates and provider networks to coordinate services Review clinical records, labs, and provider notes to inform care guidance Monitor care outcomes and adjust recommendations as needs evolve Document assessments, care plans, and family interactions accurately in EHR/CRM systems Ensure recommendations align with safety standards and regulatory requirements Participate in cross-functional meetings to improve care navigation tools and processes Contribute clinical insight to content, resources, and decision-support tools for families Required Skills Patient assessment Care planning Geriatric nursing Care coordination Clinical documentation Electronic health records (EHR) Health education Long-term care regulations Interdisciplinary collaboration Quality and outcomes monitoring
