RN Coordinator $5,000 Sign-On Bonus Paid Mileage Company Life Insurance Options Family Choice of New York is seeking an RN Coordinator to join our team. Our mission is to ensure members receive coordinated, individualized care in the most appropriate setting while supporting safe and effective transitions across the care continuum and helping each member achieve the best possible health outcomes.

Key Functions:

The RN Coordinator plays a vital role in coordinating high-quality, member-centered care for individuals with complex and chronic healthcare needs. This position is responsible for developing, implementing, and maintaining individualized care plans based on member assessments, clinical needs, provider recommendations, and established goals of care. The RN supports discharge planning, identifies ongoing care and education needs, collaborates with the interdisciplinary team and participates in quality improvement initiatives including assisting with audits and data collection. The RN Coordinator also supports utilization management and hospital diversion activities by reviewing lengths of stay, monitoring medical necessity, facilitating communication between providers and facilities, and coordinating care to ensure members receive services in the most appropriate setting. This role requires strong clinical judgment, exceptional communication and relationship-building skills, and the ability to effectively collaborate with internal and external partners to deliver coordinated, high-quality care that improves member outcomes and enhances the member experience.

Education:

RN licensed in New York State Bachelor's degree preferred Certified Case Management (CCM) preferred

Requirements:
