Job Description

The expected salary range for these positions are between $31.50-44.50 per hour. The actual compensation will be determined based on the site, hours/day of shift and other factors permitted by law. $2,500

RESPONSIBILITIES

  • Connotes Essential Functions E-1 Ensures that all indicated medical appointments are scheduled and followed up appropriately and maintains all medical records.
E-2 Ensures that all consumers are assessed according to specific program regulations. 1. Reviews medication records monthly to ensure consumers are receiving prescribed medication and that documentation of it is occurring in a timely manner. 2. Assists consumers in achieving self-medication administration as indicated. E-3 Provides 24-hour emergency on-call coverage for assigned programs, as required. E-4 Supervises AMAP's in medication administration. E-5 Assists and participates in admission screening process, as necessary. E-6 Attends meetings as required. E-7 Ensures that consumers are living in a safe and hygienic manner; provides resources for infection control. E-8 Develops and provides in-services for staff, as needed. E-9 Ensures that appropriate amounts of medication and supplies are available. E-10 Dispenses medications and provides hands-on medical treatment, as needed. E-11 In a medical emergency, obtains the proper emergency help (i.e. police, ambulance, etc.) or renders first aid and follows up with doctor's visit, if warranted. E-12 If a client requires hospitalization, coordinates and arranges admission and discharge and staff coverage along with the Residential Manager. 13
  • Reviews doctor visits and doctor's orders in a timely manner. 14
  • Keeps lines of communication open with manager, service coordinator and other staff members. 15
  • Ensure that the first aid kit is adequately supplied at all times. 16
  • Attends medical appointments and schedules follow-up appointments as per doctor's recommendations, as necessary. 17
  • Completes and sends in weekly reports, as required. 18
  • Adheres to all safety policies and procedures. 19
  • Adheres to all IGHL policies and procedures. 20
  • Obtains and maintains all required certifications through
IGHL. 21
  • All other duties as assigned.
REQUIREMENTS A. NYS LPN
license. B. Valid NYS driver's license. C. Must be able to lift at least 50 lbs. D.
