SUMMARY OF ROLE
Senior Helpers of Central Queens is looking for a dedicated and compassionate Field Nurses (RN) / Registered Nurse to provide support in our clients 'homes to help maintain their independence. The ideal candidate should be able to work independently and have a pleasant and friendly personality. Great working environment, competitive pay rates and benefits. All Field Nurses (RN) will be required to submit or undergo a full annual physical with MMR (MEASLES/MUMPS/RUBELLA) and
PPD/TB TEST
(less than a year old) as required by the state.
ESSENTIAL DUTIES & RESPONSIBILITIES
include but are not limited to: Reasonable accommodations may be made for individuals with medical limitations or disabilities to perform the Essential Responsibilities. Complete necessary documentation for nursing assessments and planning evaluation pertaining to clients, sign off on nursing assessments completed by Staff LPN. Evaluate clients as necessary to ensure client needs are consistent with the care plan. Utilize professional assessment skills to identify physical, mental, and psychosocial needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach Coordinate with management on implementing and revising operational procedures related to quality client care. Function as a resource and mentor to peers and subordinates to maintain professional nursing standards. Act on client's behalf to assure patient rights. Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitudes and practices displayed by other staff members. Respond promptly to any client complaints concerning a caregiver, document complaints, and review complaints with the Staff LPN and Director of Operations. Lead caregiver trainings onsite at Senior Helpers of Central Queens office, as needed. Perform other duties as assigned.
ADDITIONAL RESPONSIBILITIES
Complete all paperwork in a timely manner. Report any significant client changes to the office immediately. Maintain current certifications, health requirements and eligible working status as defined by the Department of Health and the Department of Homeland Security at all times. Follow all policies and procedures as outlined in the Employee Handbook for Field Staff. Perform other duties as assigned.

Demonstrated ability to operate safely in the workplace. Ability to work autonomously and understand when a superior needs to be involved in decision making. Good decision-making skills, with the ability to analyze information, evaluate results, and implement the best solution to solve problems or challenges. Self-motivated, with a dedication to keeping up to date technically.
QUALIFICATIONS
High school diploma is required. Must be a licensed Registered Nurse (RN). Must have at least two years of experience working in home health care. Must meet NY state requirements for Caregiving/RN services. Must have a valid and current state driver's license and insurance. Must have safe and reliable transportation to go to and from the client's location and/or facilities and have a vehicle available during assignments in order to assist clients with shopping needs if required. Must be available to work various shifts, including nights, weekends, and holidays.
PHYSICAL DEMANDS

Able to lift, move, and assist clients. If you feel you cannot safely lift the client, please notify the office immediately. Ability to safely assist client in and out of a vehicle and safely lift a wheelchair or walker weighing up to 50lbs into and out of a vehicle. Able to communicate with others to accurately exchange information. Able to operate office equipment such as a computer, phone, etc. Ability to work in a constant state of alertness and safe manner. Able to perform tasks such as lifting, light housekeeping, walking and driving.
