We are seeking experienced Registered Nurses to provide high-quality home care services throughout Sullivan and surrounding counties. Responsibilities Conduct admission assessments and biannual reassessments for patients. Establish and implement individualized plans of care based on nursing assessments, patient needs, and physician orders. Provide education and support to patients, families, and paraprofessional staff. Conduct as-needed nursing visits for patients requiring additional support. Communicate changes in patients' conditions, treatments, and medications promptly to the agency. Nurse Requirements Graduate of an accredited school of nursing (ADN or BSN). Current New York State Registered Nurse license and registration. Home care experience is strongly preferred . Reliable vehicle required for travel to patient homes.
