PM Key Responsibilities:

Conduct comprehensive OASIS assessments and develop personalized care plans. Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management. Evaluate patient progress and promptly communicate any changes in condition to the healthcare team and physicians. Educate patients and family members on disease processes, treatment plans, medications, and preventive care strategies. Maintain accurate, timely documentation using Home Care Home Base (HCHB). Work collaboratively with physical therapists, occupational therapists, medical social workers, and home health aides. Participate in case conferences, staff meetings, and ongoing educational activities. Ensure compliance with all documentation, clinical, and regulatory standards. Travel daily within the assigned territory to patient homes (mileage reimbursement provided).

Required Qualifications:

Active RN license in good standing in the state of practice. At least 1 year of recent field-based Home Health experience. Proficiency with Home Care Home Base (HCHB) electronic documentation. Demonstrated skill with OASIS assessments. Reliable transportation, current drivers license, and auto insurance.
