Pride Health is hiring a Mental Health & Psychiatric Registered Nurse to support our client's healthcare facility in Bronx, NY. This role is ideal for a nurse with strong case management skills and experience working with individuals who have mental health or psychiatric needs. The RN will play a key role in supporting residents' overall health and wellness through care coordination, medical follow-up, wellness monitoring, and documentation. About The Program Supported:

This position is based within a supportive housing program serving approximately 86 residents. Residents live independently in their own apartments and are generally able-bodied. While some individuals may have mental health diagnoses or other challenges, the population does not require direct support professional (DSP) services.

Duties:

Coordinate and assist residents with medical appointments and follow-up care

Provide wound care and routine nursing support as needed

Conduct wellness assessments and routine health checks

Follow up with residents after hospital visits and assist with care transitions

Maintain accurate and timely documentation of all resident interactions, medical follow-ups, and related services

Utilize the AWARDS electronic health record/charting system

Collaborate with program leadership and community healthcare providers to support resident well-being

Requirements:

Active New York State Registered Nurse (RN) License and Registration

BLS/CPR Certification through the American Heart Association (AHA)

Professional Malpractice Insurance

Associate's Degree in Nursing or higher

2 years of experience working with mental health and/or psychiatric populations

Strong nursing case management and care coordination skills. #
