A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan.   • This is a PD Saturday Position Coordinates the care and services of patients across the continuum of their hospitalization.
The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes. Qualifications BSN Graduate of NLN Accredited School of Nursing Required or MS Masters Degree from an accredited school of Social Work Required 1-3 years Experience in an acute care environment Required Ability to read, write, and communicate in English. Strong clinical, organizational, and interpersonal skills. Registered Nurse New York
  • New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse. Required Licensed Clinical Social Worker
  • New York State Board of Social Work within 180 Days Required CCM
  • Certified Case Manager
  • Commission for Case Manager Certification Certification in Case Management required within 1-1/2 Yrs Required PRI
  • Patient Review Instrument Cert
