Queens, NY Salary:
$55.92/hour + Differential + 1199 Union Benefits Make a Direct Impact on Patient Care and Outcomes Jamaica Hospital Medical Center, a 384-bed community teaching hospital serving one of New York City's most diverse populations, is seeking an experienced RN Case Manager to support coordinated, high-quality patient care across the continuum. In this role, you will work closely with physicians, nurses, and interdisciplinary teams to ensure safe, effective, and timely care planning, discharge coordination, and resource utilization. What You'll Do Conduct comprehensive patient assessments to identify care needs and discharge planning requirements Coordinate care across multidisciplinary teams, including physicians, social workers, and ancillary services Develop and implement individualized care and discharge plans Monitor patient progress and adjust plans to ensure optimal outcomes Facilitate timely discharges while addressing barriers to care Ensure accurate and compliant documentation in accordance with hospital and regulatory standards Perform utilization review and collaborate on appropriate level-of-care determinations What We're Looking For Active New York State RN License (required) Bachelor of Science in Nursing (BSN) preferred Minimum of 1-2 years of case management, care coordination, or related clinical experience in a hospital setting Strong knowledge of: Discharge planning Care coordination Utilization review Excellent communication, critical thinking, and organizational skills Ability to work effectively in a fast-paced, team-oriented environment Experience with EPIC or similar electronic medical records system preferred Why Join Jamaica Hospital Competitive hourly rate of $55.92 plus shift differentials Full 1199 Union Benefits Package , including health, pension, and paid time off Stable, union-backed position with strong job security Collaborative, interdisciplinary care environment Opportunity to build expertise in case management and patient care coordination Meaningful work serving a diverse Queens community Apply Today Join a team committed to delivering high-quality, patient-centered care while supporting safe and effective transitions across the healthcare continuum.
