SUMMARY:
Provide skilled nursing care to patients in their homes under physician orders and the direction of the Registered Nurse Case Manager. Collaborate with the interdisciplinary care team, perform patient assessments, administer treatments, educate patients and families, maintain accurate documentation, and communicate changes in patient condition to ensure high-quality, coordinated care.

Monday-Friday, 8:00 AM-4:30 PM. Participation in the on-call rotation is required approximately 1-2 days per month, 3 Holidays a year Sign On bonus : up to $15,000
Location:
Monroe County Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
Education:
New York State Licensed RN Experience:
One year of experience as a clinical nurse in a hospital or other direct patient care clinical setting required
Licensure:
New York State Licensed RN Key Responsibilities Provide skilled nursing care, including patient assessments, medication review, wound care, IV therapy, and other clinical interventions in the home. Develop and update individualized care plans in collaboration with patients, families, physicians, and the interdisciplinary care team. Educate patients and caregivers on disease management, medications, self-care, and preventive health strategies. Complete accurate and timely clinical documentation in compliance with regulatory and organizational standards. Coordinate care, communicate patient status, and connect patients with appropriate community resources. Support quality, safety, and patient satisfaction initiatives while meeting productivity and documentation expectations. Participate in continuing education, quality improvement activities, and other duties as assigned.
Salary Range:
$73,652 to $103,854 #URM1
