Functions independently to perform comprehensive, age-appropriate history and physical examination for healthy, acute, critical, and chronically ill patients. Identifies and documents normal and abnormal findings Recognizes and considers age-specific needs of patients. Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs including laboratory tests, xrays, EKGs with assistance of collaborating physician as needed. Performs designated procedures according to current standards of care (AAP) and with permission of collaborating physician. These include, but are not limited to, suturing, stapling, I & D, wound care, fb removal, splinting. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities including certain vaccinations offered in PAUC setting. Providers with DEA certifications can prescribe schedule II-V controlled substances in US alongside a collaborating physician; this is not a requirement of NP and PAs employed at PAUC. Monitors the effectiveness of interventions and modifies treatment plan Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary Facilitates the patient's transition within and between health care settings: referring, transferring, and discharging patients. Collaborates with community primary care physicians and specialists as needed to make decisions about treatment plan and appropriate follow up care. Makes appropriate referrals. Initiates arrangements for transfers- discusses transfer with PCP, gives report to accepting Emergency Department or clinic, completes appropriate paperwork to accompany patient. Communicates with patient's primary care physician . Assures that completed patient progress notes are sent to patient's PCP within 24 hours of visit. Consults primary care provider on call when appropriate to discuss care of patient. Orders equipment necessary for care at home and assures that training for their safe use is completed prior to discharge (i.e. Aerochamber, nebulizer, splints/immobilizers, crutches) . Educating patients on treatment plan, disease prevention and positive health and lifestyle choices. Communicating with PAUC nurses and medical assistants so that they can create comprehensive, accurate discharge instruction packets. Effectively communicates and interacts with patients, families, staff and members of the community. Self motivated to acquire Electronic Medical Record (Medent) skills necessary to ensure efficient and thorough documentation in every patient medical record. Provide professional guidance and supervision to medical assistants, RNs, LPNs and clerical staff , volunteers as appropriate
