Schedule:

Monday Friday, 8:00 AM 5:00 PM Your caseload will average 30 points per week, typically translating to 35 patient visits daily. Daily travel within your territory is required, with mileage reimbursement provided.

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop personalized care plans.

Deliver skilled nursing interventions:

wound care, medication administration, IV therapy, and management of chronic conditions. Monitor patient progress, identify changes, and promptly communicate with physicians and healthcare team members. Educate patients and families about care plans, medications, and preventive care strategies. Ensure accurate and timely documentation using Home Care Home Base (HCHB). Collaborate closely with therapists, medical social workers, and home health aides for integrated care. Participate in case conferences, meetings, and ongoing education. Maintain adherence to all relevant regulatory and internal standards for documentation and patient care.

Qualifications:

Active RN license (state of practice), in good standing. At least one year of recent home health experience; field experience preferred. Proficient with Home Care Home Base (HCHB) and OASIS assessments. Reliable transportation, current drivers license, and valid auto insurance.
