Job Description

MDS Coordinator- RN OR LPN Join our team at Cedars of Lebanon, where we are committed to resident satisfaction and providing services that recognize the specific needs of each person in our care! We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon, As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.
Responsibilities:
Monitor and guide the completion of assessments in a timely manner Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtaining receipt of validation Schedule and facilitate care plan conferences and monthly QM meetings
Manage Care Planning Process Mandatory Qualifications:
Must hold a Nursing Degree from an accredited college or university Current, unencumbered, active RN license or LPN and CPR At least 3 years' experience in Long Term Care as a
MDS Coordinator RAC-CT Preferred Desired Skills and Qualities:
Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement Excellent written and verbal communication skills and critical thinking abilities Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility Ability to organize and prioritize Why join our team at Cedars of Lebanon?
