Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members. Review and evaluate prior and current medical histories, existing medical conditions, and medication lists. Qualifications:

Active Nurse Practitioner license in Ohio Current CPR certification (BLS) Board Certification by AANP or
