A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.
Full-Time Position Overview We are seeking an experienced and compassionate Hospice RN Case Manager to join our growing hospice team. The ideal candidate will have prior hospice case management experience and be comfortable providing care in patients' homes while coordinating services across the interdisciplinary care team. This role is responsible for managing a caseload of hospice patients, developing individualized care plans, providing skilled nursing assessments, and ensuring patients and families receive high-quality end-of-life care. Key ResponsibilitiesDaily Patient Care & Case Management Conduct comprehensive nursing assessments for hospice patients in their homes. Develop, implement, and update individualized plans of care based on patient needs and physician orders. Manage a designated hospice caseload and monitor patient status throughout the continuum of care. Perform skilled nursing visits and evaluate symptom management, pain control, and overall patient comfort. Educate patients, caregivers, and family members regarding disease progression, medications, treatments, and end-of-life care. Coordinate care with physicians, social workers, chaplains, home health aides, and other interdisciplinary team members. Respond to changes in patient condition and implement appropriate interventions. Ensure compliance with hospice regulations, clinical standards, and company policies. Documentation & Coordination Complete accurate and timely documentation utilizing Homecare Homebase (HCHB). Review medication regimens and communicate recommendations to physicians as appropriate. Participate in care conferences and interdisciplinary team meetings. Coordinate admissions, recertifications, and other required clinical assessments. Maintain communication with referral sources, families, and healthcare providers. Travel Travel between patient homes throughout the assigned territory. Approximately 50% to 75% travel required during the workday. Regular travel to multiple patient locations during each shift. Required Qualifications Active, unrestricted Registered Nurse (RN) License Current CPR Certification Previous Hospice RN Case Management experience Experience using Homecare Homebase (HCHB) Valid driver's license, reliable transportation, and proof of auto insurance Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Preferred Qualifications Candidates who stand out will possess: CHPN (Certified Hospice and Palliative Nurse) certification Experience managing high-volume hospice caseloads Strong knowledge of Medicare hospice regulations and compliance requirements Exceptional time management and organizational skills Experience collaborating with interdisciplinary care teams Proven ability to work independently while maintaining quality patient care Compassionate bedside manner and strong relationship-building skills with patients and families
