The Role:

As the Admissions Nurse, you set the standard for the care the patient will receive. You are the clinical expert conducting the initial assessment and establishing the plan of care.

Key Duties:

Performing "Start of Care" visits, completing accurate OASIS documentation, ensuring medication profiles are precise, and conducting home safety assessments. You'll also serve as a key consultant for physicians and the office, keeping everyone aligned on patient needs.

What You Bring:

Current RN License in the state of employment. A valid driver's license and current auto insurance. Prior home health case management experience (preferred). High attention to detail and a commitment to clinical excellence.
