The Registered Nurse Case Manager provides a wide range of nursing care to patients in the home care setting. The RN Case Manager renders professional nursing care by assessing, developing, implementing and evaluating home care needs. This position will serve Trumbull County. Looking for full time and part time candidates.
Responsibilities:
Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients. Assess, plan, direct and evaluate each patient's care plan to ensure it meets the patient's needs. Manage emergency situations in a prompt, precise, and professional manner. Complete medication administration and treatment needs for all patients assigned. Provide nursing care in accordance with applicable regulations and company policies. Prepare and maintain accurate and complete clinical and progress notes on a timely basis Provides supervision and oversight to patient care. Utilize the Electronic Visit Verification system. Prepares physician orders as applicable. Participates in continued education programs, patient care conferences and staff meetings as scheduled. Performs nursing assessments (initial and ongoing). Maintains excellent assessment skills and clinical knowledge and expertise. Notifies the physician and branch management of significant changes in patient status and obtains changes in orders as necessary. Assumes responsibility for ongoing interdisciplinary assessment of the patient. Participates in branch on-call rotation as assigned. Follow organizational policies and procedures regarding infection control, disposal of hazardous waste and use of protective devices as evidenced by documentation. Performs other job-related tasks and duties as assigned.
Qualifications:
Education/Experience Desired o Nursing supervisory/leadership experience preferred o Six months' experience in home care preferred. License Requirements o Active RN state license o Valid drivers' license o Current CPR certification
SKILL REQUIREMENTS
Ability to work flexible hours as required to meet identified patients' needs. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient. Able to work independently. Must possess a strong commitment to the goals, mission, and philosophy of the organization. Understands the concepts of home health and the role of the RN as a member of a full discipline health care team.
