The Role:

As our RN Case Manager, you are the heartbeat of our patient care. You aren't just performing visits; you are the primary point of contact for patients, families, and physicians. Key Duties:

Managing client care, coordinating with physicians, overseeing OASIS collection, managing medication profiles, and participating in weekly case conferences.

What You Bring:

Current RN License in the state of employment. A valid driver's license and current auto insurance. A commitment to compassionate, high-quality nursing.
