Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage plan members Travel to patient residences within Pontotoc County for evaluation Review and document prior and current medical conditions, medical history, and medication lists Provide education to patients regarding medical conditions, health maintenance, and medication management Identify and communicate care gaps, offering actionable recommendations for addressing those gaps Focus on assessmentsno treatment plan development or prescribing of medications needed

Qualifications:

Nurse Practitioner licensure in Oklahoma Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited school of nursing At least six months of recent NP experience preferred Strong communication skills and the ability to work independently As part of this role, you will receive competitive pay at $80$90 per evaluation, along with a mileage stipend and compensation for administrative time related to your visits.
