Job Description

Position Summary The MDS Coordinator is responsible for coordinating and overseeing the accurate, timely completion and submission of the Minimum Data Set (MDS) assessments in accordance with applicable federal and state regulations. This position works closely with nursing, therapy, dietary, social services, and other interdisciplinary team members to ensure comprehensive resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement. Essential Duties and Responsibilities Coordinate the completion, review, and submission of MDS assessments within required timeframes. Ensure accurate documentation of resident clinical conditions, functional status, treatments, and services. Lead or coordinate interdisciplinary team meetings related to MDS assessments and care planning. Develop, update, and maintain individualized resident care plans based on assessment findings. Collaborate with nursing, therapy, dietary, social services, and other departments to obtain accurate supporting documentation. Review medical records for completeness, accuracy, and consistency with MDS information. Identify documentation discrepancies and communicate concerns to the appropriate department or supervisor. Monitor assessment schedules, including admission, quarterly, annual, significant change, and other required assessments. Assist with identifying appropriate diagnoses, services, and clinical documentation to support accurate reimbursement. Maintain current knowledge of CMS regulations, RAI Manual requirements, and applicable state and federal guidelines. Participate in quality assurance and performance improvement activities. Assist with survey preparation and respond to MDS-related requests from regulatory agencies, auditors, and other authorized parties. Maintain confidentiality of resident information in accordance with HIPAA and facility policies. Utilizing current EHR and/or third party software, ensure successful submission/ transmission of required MDS information. Perform other duties as assigned by the Director of Nursing or Administrator. Qualifications Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Oklahoma, as applicable to facility requirements. Previous experience in a skilled nursing facility, MDS coordination, or long-term care nursing preferred. Knowledge of the MDS assessment process, RAI requirements, and care planning. Strong clinical assessment, documentation, organizational, and communication skills. Ability to work independently and collaboratively with an interdisciplinary team. Proficiency in electronic medical records and MDS-related software. Physical and Work Requirements Ability to work in a skilled nursing facility environment. Ability to review medical records, attend meetings, and communicate effectively with residents, families, and staff. Ability to meet the physical and mental demands of the position, with reasonable accommodations as applicable. Pay:
$30.00 - $35.00 per hour Expected hours: 40.0 per week
