Deliver essential care in your community as a skilled Nurse Practitioner specializing in home health. In this per diem role, you will perform home health risk assessments throughout Pottawatomie County, including Shawnee and surrounding areas such as Tecumseh and McLoud. Key Responsibilities:

Conduct annual health risk assessments for Medicare Advantage members in their homes Review and document prior and current medical histories, conditions, and medication usage Educate patients on medical conditions and prescribed medications Identify gaps in care and provide actionable recommendations Visits typically last about 45 minutes; types of visits may vary (annual health assessments, post-discharge check-ins) No requirement to develop treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner license in Oklahoma Board Certification through AANP or ANCC Current CPR (BLS) certification Masters degree in Nursing (MSN) from an accredited institution At least six months of current Nurse Practitioner experience preferred Enhance patient outcomes while enjoying autonomy and a supportive structure.
